Systematic approaches to preventing complications in laser and energy-based device (EBD) treatments

Safety is the top priority when using lasers and energy-based devices (EBD). To ensure this, we have developed the "POEM" framework.

Prediction: Identify potential risk factors and inform patients accordingly.

Observation with Monitoring: Closely observe and monitor patients throughout the treatment. It is essential to determine the endpoints precisely.

Equilibrium and Balance: Maintaining a balance between safety and efficacy is helpful. We will present detailed examples and techniques will be presented.

Management: Respond to manage any complications that may occur. For example, good-quality wound treatment will prevent further worsening and the formation of hypertrophic scar/ keloid. Further treatment for paradoxical hypertrichosis after laser epilation and paradoxical adipose hyperplasia is also essential.

With the POEM framework, we can systematically prevent and manage complications. This presentation will provide detailed examples to illustrate these fundamental principles.

Botulinum toxin injection is a fundamental and effective treatment option in anti-aging procedures. Since 1992, when Drs. Jean and Alastair Carruthers published the first paper on the treatment of glabellar lines using botulinum toxin, it has become widely used for wrinkle treatment. Botulinum toxin works by inhibiting acetylcholine release at the neuromuscular junction, which prevents muscle contraction. This relatively simple procedure offers the major advantage of allowing patients to return to their daily routines immediately after treatment, contributing to its growing popularity. However, with the increasing prevalence of the procedure, there has also been a rise in side effects, often due to inexperienced injection techniques. To ensure both safety and effectiveness, it is crucial to have a thorough understanding of the anatomy related to botulinum toxin injection and to apply proper injection techniques based on this knowledge. In this lecture, I will cover the basic principles for safe and effective botulinum toxin injections, including facial muscle anatomy, recommended dosages for each injection site, precautions during injections, and tips for addressing various issues encountered in daily clinical practice.

Etiology and management of delayed inflammatory response with hyaluronic acid fillers

Background:
Hyaluronic acid (HA) fillers are widely used for facial volumization and contouring due to their biocompatibility, reversibility, and safety profile. However, delayed inflammatory responses (DIR) can occasionally occur weeks to months after injection, presenting as swelling, erythema, nodules, or induration. The etiology of these reactions is not fully understood, and management can be challenging, especially in aesthetic practice where patient satisfaction is crucial.
Objective:
This presentation aims to explore the etiology of delayed inflammatory responses following HA filler injections, review contributing factors such as immunological triggers and environmental influences, and propose effective management strategies to mitigate these adverse effects.
Conclusion:
The etiology of delayed inflammatory responses with HA fillers appears multifactorial, with immune triggers and biofilm formation playing significant roles. Prompt recognition and tailored management with corticosteroids, hyaluronidase, and, when indicated, antibiotics can effectively resolve symptoms and maintain patient satisfaction.

Understanding vascular complication of filler injection

This lecture will discuss the serious dangers of vascular occlusion, which can result in skin death, blindness, or even stroke, being a crucial risk that is associated with filler injections. As a board-certified dermatologist with years of experience, I have been able to diagnose and treat several cases of complications related to vascular occlusion that have led me to develop specialized procedures and techniques aimed at minimizing the risks associated with these treatments. This presentation will feature an in-depth analysis of my own professional experience, as well as the latest research, clinical studies, and case reports on vascular complications related to filler injections. The goal is to equip medical practitioners with vital knowledge that would aid in reducing the vascular complications that can arise from such procedures. By the end of the lecture, attendees should have a deeper understanding of the importance of careful consideration of factors such as anatomical variations, injection technique, and injection-site selection, to prevent serious complications associated with filler injections.

Dr. Shiou-Han Wang is the superintendent of Skin168 Institute of Dermatology in Taipei and holds adjunct positions at National Taiwan University Hospital and Doctors' Doctor Clinic. He also serves on the Review Committee of the Taiwan Food and Drug Administration(TFDA). He is also the executive director/ directors of Taiwanese medical societies related to laser and aesthetic surgery, including CSCSM, LMSTW, TSDAS, and TMIAS.
Dr. Wang earned his MD and MSEE from National Taiwan University. His research focuses on laser, radiofrequency, and ultrasound in dermatology. He specializes in dermatologic surgery, laser treatments, and acne and scar therapy.
He has received awards for his research and is a reviewer for several academic journals. Dr. Wang is also a regular speaker at national and international dermatology and aesthetic conferences.
