Targeted cne therapy: Light-absorbing gold microparticles combined with long-pulsed 1064nm Nd:YAG laser

Introduction:<br/>Acne vulgaris is a common dermatological condition characterized by inflammation and sebaceous gland hyperactivity. Current treatment strategies include topical and systemic therapies, but limitations such as side effects and resistance have driven the need for novel approaches. Light-absorbing gold microparticles activated by long-pulsed 1064 nm ND Yag laser therapy have emerged as a promising non-invasive method for targeting acne lesions. This study aims to compare the efficacy of two concentrations of gold microparticles combined with laser therapy in treating moderate to severe acne vulgaris.<br/><br/>Material and methods:<br/>A total of 10 patients aged 16-35 years, diagnosed with moderate to severe acne vulgaris, were recruited for this study. Patients had not received any acne treatments for at least three months prior to, during, and one month following the treatment sessions. Group 1 (n=4) received 1-2 cc of a gold microparticles solution, while Group 2 (n=5) received 2.5-3.5 cc. Treatment involved the application of the gold microparticle solution, enhanced with sonophoresis, followed by 1500 shots of the long-pulsed 1064 nm ND Yag laser. Sessions were conducted three times, with a two-week interval between each. Skin condition was evaluated using conventional photography and Janus skin analyzer (Korea) before the first session and two weeks after the final session.<br/><br/>Results:<br/>Group 2, treated with a higher concentration of gold microparticles (2.5-3.5 cc), showed significantly better outcomes than Group 1 (1-2 cc). Improvements included a marked reduction in inflammatory acne lesions, post-acne erythema, and subjective reports of decreased skin oiliness. No new acne lesions were observed, and no recurrences were reported during the follow-up period. In contrast, Group 1 showed moderate improvement, with some reduction in acne lesions and erythema, but less pronounced than in Group 2.<br/><br/>Discussion:<br/>The study’s findings suggest that a higher concentration of gold microparticles, when combined with long-pulsed 1064 nm ND Yag laser therapy, provides a more effective treatment for moderate to severe acne vulgaris. The enhanced penetration and absorption of the gold microparticles may account for the observed superior clinical outcomes. Additionally, the lack of adverse events and the high patient satisfaction underscore the potential of this therapy as a safe and effective option for acne management.<br/><br/>Conclusion:<br/>This study demonstrates that using a higher concentration of gold microparticles with a long-pulsed 1064 nm ND Yag laser is more effective in reducing acne lesions and improving skin appearance compared to a lower concentration. Further research with larger sample sizes and longer follow-up periods is recommended to confirm these findings and explore the underlying mechanisms.

Professor Firas is one of the UK's most prolific and well-published dermatologists and one with a renowned international standing.​ He is a highly experienced and respected Consultant Dermatologist with nearly 20 years of medical experience and trained in some of the UK's top hospitals including the largest Dermatology Department in Europe (Manchester) and the world-renowned St. John's Institute of Dermatology at St. Thomas’ hospital where he completed a prestigious Laser and Mohs Surgical Fellowship there and continued to work as an Honorary Consultant for several years.  ​ Currently in addition to his private practice he is involved in academic research and international teaching and training of doctors and was awarded the "Global Excellence Award" for the title consultant dermatologist of the year 2019 in London given his services and commitment to dermatology.
